This website is provided by Mayden Healthcare who is the contracted supplier for the clinical patient management system we use in Talking Therapies. This website is secure to use and the information collected within the referral form is kept strictly confidential. It is only used for the purpose of transferring your information from the referral form into the patient management system. For further information on how we use your information including your rights, please visit our full privacy notice.

The information gathered on your online form will help us identify if this service is right for you. It will take approximately 30 minutes to complete, depending on how much information you choose to share at this stage.

Unfortunately, you will not be able to save the form as you go along, only at the end (this is to keep your information safe), so please make sure you have plenty of time to complete it in one go.
