If at all possible we want to help people stay out of hospital and receive any care they may need within their local community or at home.
We have lots of specialist services to provide this including community nurses, health visitors, rehabilitation services, podiatry and sexual health. You can find lots of information on all our services by visiting our services page.
If required, adult social care can be provided by Northumberland County Council or North Tyneside Council, depending on where you live.
