If you are overcoming an illness or adapting to a disability, our occupational therapists can support you to regain your independence.
We can do this in a number of ways such as through advice, arranging special equipment or making adaptations to your home.
We also offer support following a hospital stay if you need additional care to recover. This is called ‘facilitated discharge’. It is a therapy and nursing service which supports patients home from hospital.
This is jointly delivered by NHS and adult social care rehabilitation service.
