Specialist support for memory concerns
Providing assessment, diagnosis and guidance for individuals aged 65+
We are part of Mental Health Services for Older People (MHSOP), offering specialist support for those experiencing memory difficulties.
Our team includes:
- Mental health nurses & healthcare assistant
- Consultant psychiatrist & medical team
- Clinical & assistant psychologists
- Occupational therapists
- Administrative & domestic support
What we do
We offer person-centred assessments for people with memory problems who do not already have a dementia diagnosis.
Our aim is to:
- Understand the nature of memory concerns
- Provide appropriate diagnosis
- Offer guidance and referrals for further support
We work closely with:
- Community Mental Health Teams
- Younger person’s dementia service
- Nursing home & inpatient teams
Referrals are usually made by your GP, but can also come from other health professionals.
What the referral should include:
- Details of memory concerns (onset, duration, symptoms)
- Any known risks or safeguarding issues
- Your consent to be referred
- A recent cognitive screening (e.g. 6CIT or GPCOG)
- Blood tests completed in the last 6 months:
- FBC, ESR, CRP, U&Es, Calcium
- HbA1c, LFTs, TFTs, B12, Folate
Referral forms are available to professionals via the Trust intranet.
After referral, you’ll be offered an appointment for a memory assessment. This may involve:
- Talking to you (and family, if appropriate)
- Cognitive tests and questionnaires
- Medical and psychological assessments
- Feedback and follow-up planning
