We are running a Connecting Care for Children MDT (multidisciplinary team) pilot in Northumberland.

The GP practices involved are:

  • Bedlingtonshire Medical Group
  • The Gables Medical Group
  • Guidepost Medical Group
  • Seaton Park Medical Group

 

We are trialling a new way to support children and young peoples’ health needs. This includes:

  • Chronic pain
  • Neurological disorders (affecting the brain, spinal cord, and nerves)
  • Severe mental illness
  • Physical disabilities

We are improving access to local, joined-up child health services and reducing the need for unnecessary hospital visits.

 

The model supports the NHS Long Term Plan. It follows successful national examples, where this new way of working has shown:

  • Reduced A&E attendances
  • Positive family feedback
  • Better joined-up care

Your GP may talk about the Connecting Care for Children MDT pilot to improve support for your child.

It brings all health professionals involved in your child’s care together. This includes:

  • GPs
  • Consultants
  • Health visitors
  • Primary Mental Health Team
  • Early years teams
  • School nurses
  • Paediatric dietitians
  • GP / paediatric trainees
  • Northumberland Family Hubs and Early Help

The pilot brings more specialist expertise on child health within easy reach. Paediatric consultants from the local hospital join GPs and other health and care professionals involved in your child’s care to work together more closely.

This includes:

  • GPs having direct access to children’s health specialists at our hospitals in Northumberland, with a phone line and email for advice
  • Specialist outreach clinics and multidisciplinary meetings with GP surgeries every month
  • Building relationships and working with champions in the community to improve the health of local populations

  • Fewer appointments to attend
  • The right person will see you in the right place, at the right time. Often this will be at your GP practice
  • Each patient and their family get one agreed management plan
  • Primary care professionals with enhance skills can deal with child health concerns in the community
