We are a team of nurses, exercise professionals, physiotherapists and admin. Our aim is to support and encourage people with a heart-related illness on their road to recovery and to work towards healthier lifestyles.

We aim to provide a personalised approach to your care, acknowledging physical, social and psychological needs.


You will be referred to Cardiac Rehabilitation by your GP or hospital team following discharge.

We offer cardiac rehabilitation across Northumberland and North Tyneside.

We can support home-based Rehabilitation or you can join us at one of our venues below.

  • Willowburn Leisure Centre, Alnwick
  • Ashington Leisure Centre
  • Concordia Leisure Centre, Cramlington
  • Wentworth Leisure Centre, Hexham
  • Waterworld Leisure Centre, Prudhoe
  • Parks Leisure Centre, North Shields
  • Westmoor Community Centre.

  • A heart attack
  • Coronary angioplasty, balloon or stent.
  • Heart surgery including coronary artery bypass graft surgery and valve repair or replacement.
  • We offer education and support to enable healthy lifestyle choices,
  • Such as smoking cessation, healthy eating and weight management.

 

We provide a programme of health advice, psychological support and exercise to reduce the risk of a further cardiac event.

It is delivered in four phases and during each phase there will be a qualified nurse available to give you all the advice or information you need.

Phase 1

In hospital you will be given advice about any risk factors and guidance about your recovery. Information leaflets about the service are also provided.

Phase 2

After you come out of hospital you will be contacted by one of our cardiac rehabilitation nurses. We will arrange a suitable time to carry out an assessment, in clinic or at home, and offer health education and support on a one to one basis. We will also give you more information about the next phases of the programme. If you haven’t heard anything after the first week of returning home from hospital, please call us on the contact details below.

If you have been issued with The Heart Manual Your Rehab Nurse will be able to guide you through and support you with this.

Phase 3

This normally starts between two and four weeks after your heart event and lasts for about six weeks.  We provide a programme of health education, exercise and relaxation.

This will be delivered in group sessions, normally at a local leisure centre.

You may benefit from being enrolled onto the British Heart Foundation DVD to use at home, your Nurse will offer support and guidance.

Phase 4

After you have finished phase 3, we will talk to you about the support available to help you stay healthy and you may be able attend our phase 4 fitness classes.
