Following a stay in hospital you may need extra support or rehabilitation. The intermediate care service provides this support helping you to recover faster from your illness and to make sure that you can live as independently as possible once you get out of hospital.
The service also helps to prevent unnecessary hospital admissions and ensures that people who do not need to remain in hospital can move home or to another facility where they can receive support and rehabilitation.
The service works closely with North Tyneside Council to ensure that intermediate care links with complimentary services such as rehabilitation wards, social services resource centres, specialist medical services and short and long-term care at home services.
Initially a full assessment of your needs is undertaken and from that a coordinated package of care and rehabilitative treatment is provided. Depending on individual need, support can be provided in your home, at a community hospital or in a nursing home.
