If you have a long term condition, complex health needs or have been admitted to hospital several times recently and need help to stay well at home our community matrons can support you.
Our community matrons are highly experienced senior nurses who will work closely with you and your doctor to plan and organise care at home.
As well as providing your nursing care we will act as your case manager – the single point of contact for advice and all your care and support needs.
If you think you would benefit from having a community matron please speak to your GP or district nurse.
If you are able to get to your GP surgery you can also make an appointment to see your community matron there.
If you have a long term condition, we will work closely with you to manage your condition at home for as long as possible.
We can help with spotting early warning signs if your health starts to deteriorate, for example ensuring you have the right care and medication when you need it and/ or if any difficulties arise affecting your ability to cope at home we can review your care to prevent you going into hospital.
You can contact your community matron through your GP from 8am-5pm. If you call after 4.30pm you will be contacted the next day.
