If you are suffering from coronary heart disease we provide a range of services including rehabilitation after a heart attack or heart surgery, care if you’ve had heart failure and home visits if you’ve been discharged from hospital.
You can be referred into cardiac rehabilitation programmes following a heart attack, angioplasty/stent or heart surgery through your hospital doctor or GP.
You can see the community matron for heart failure services through your GP, consultant cardiologist or district nurse.
