We provide a range of specialist services in peoples’ homes, in the community and in our hospitals in North Tyneside to support people living with dementia and their families to stay active, healthy and safe.
Our health services in North Tyneside provide specialist treatment in people’s homes and also in our day hospitals, memory clinic and at North Tyneside General. This includes carrying out assessments, screening for memory problems and dementia, prescribing medication and providing information, aids and adaptations to help people to live at home.
Specialist health services for people living with dementia in Northumberland are provided by Northumberland Tyne and Wear NHS Foundation Trust.
We provide social care in Northumberland including social care assessments of need for people with dementia and for people looking after someone with dementia, specialist day services, access to practical help with daily living, and assistive technology called telecare to help people live at home safely.
Staff teams which support people with dementia include social workers, care managers, therapists, health promotion specialists, community nurses, district nurses, Macmillan and Marie Curie nurses.
