Community rehabilitation

The community rehabilitation service offers a wide range of rehabilitation and support services for people who may have care needs relating to complex or chronic illnesses, a stroke, brain injury, or a fall. This service aims to provide short term support to help people to recover skills and confidence by providing extra help to live independently, and remain at home for as long as possible.

Clinics

The service is delivered in your home following a referral from a health or social care professional such as a nurse, GP or care manager.
 
The service is not available at weekends. If support is required at the weekend, links to other services, such as home care, START (Short Term Assessment and Reablement Service) or the Rapid Response team can be made.
 

Your care

You will be fully assessed at home to find out what you can do and what you would like to be able to do. We will work with you to decide the rehabilitation programme which is right for you which will:

  • State what achievements are most important for you, and agree goals to meet these
  • Provide a quality service within your own home
  • With your permission we will communicate with all those involved, including family, carers and other professionals
  • Provide advice and support
  • Regularly review goals

The team  

The service is provided by a team of professionals which may include: 
  • A physiotherapist
  • An occupational therapist
  • A speech and language therapist
  • Rehabilitation workers
  • Technical instructors
The service works alongside a range of services and professionals including care managers, home care services and other community services.
